New Patient Forms
Please take the time prior to your Initial Exam appointment to print out and complete the Patient Information, Medical History and Insurance Benefits Confirmation Forms. If you are unable to do so, please arrive 15 minutes early to your scheduled appointment, and we will be happy to provide you with these necessary forms. We look forward to meeting you!

Insurance
Although we are not listed as a preferred provider on insurance plans, we do file and accept payment directly from most insurance companies. We ask that you become familiar with your benefits by completing our Insurance Benefits Confirmation page prior to your Initial Exam. By completing this form in it's entirety, we will be able to quote your out of pocket expenses based on possible treatment options and set up a payment plan for your portion.
Financial Arrangements
We believe that financial considerations should not be an obstacle to obtaining a healthy, beautiful smile. Being sensitive to the fact that people have different needs in meeting their financial obligations, we provide payment options at zero percent interest financing. Generally, an initial down payment (based on your treatment options) is due when you decide to begin treatment and then monthly payments are extended over a period of time.

For your convenience, we can set up your payments so that they are automatically charged to your credit card monthly or provide you with a coupon book. Payment may also be paid here through our website.
Appointment Scheduling
Patient Hours:
- Monday, Wednesday & Thursdays 8:00am - 5:00pm
- We are closed for lunch from 12:30pm - 2:00pm
Office Hours:
- Monday – Thursday 8:00am – 5:00pm
Routine appointments are scheduled every 3 to 8 weeks during active treatment.
Should you need to reschedule an appointment, we kindly ask for 12 hours notice.
We realize that afternoon appointments are preferred due to school and/or work; however, we are unable to see all of our patients during this time. In order to serve our patients in the most efficient way possible, lengthy procedures such as placing of braces, removing of braces are scheduled in the morning; and shorter appointments such as active adjustments and retainer checks are scheduled in the morning or afternoon.
